GMPを勉強しよう-2-リスクマネジメントで指摘される？
たまに「リスクで指摘された例はあるか？」と聞かれることがある。リスクマネジメントの導入が言われてから久しいだけに、気になるところだ。
FDAのWarning Letterに関して言えば、ダイレクトに指摘された例はない。Warning Letterの性質上、そのようになっているのだと思う。Warning LetterはCGMPの条項を引用するので、CGMPの条文に記載されていない「リスクマネジメント」は直接指摘できないためだろう。
指摘事項の中に引用されているかを探してみると、それらしいものは出てくる。
“Your firm became aware of this contamination in January 2012 and completed risk assessments to determine the impact on the quality of (b)(4) manufactured using the contaminated solvents on April 19, 2013. “
しかし、この指摘事項のタイトルは"1. Failure to fully investigate critical deviations. “なので、リスクアセスメントが不十分だというダイレクトの指摘ではない。
視点を変えてみると、FDAはいくつかのWarning Letterの中で、回答書にリスクアセスメントを実施して報告するように提案している。たとえば、
“A risk assessment of the potential effect of the observed failures on the quality of drug products. As part of your risk assessment, determine the effects of your deficient documentation practices on the quality of the drug product released for distribution…"
これは、FDAのWarning letterに書かれた回答要求に関するコメントである。その他、
“In your response, provide a detailed action plan for an investigation into the impact on the quality of previously released product, along with your risk assessment. “
“In your response to this letter, provide a comprehensive evaluation of the extent of the deletion and destruction of records, a risk assessment regarding the potential impact on the quality of products, and a comprehensive corrective and preventive action plan."
などがある。"In your response…"として求めていることがわかる。
ということで、直接指摘されることはないにしても、FDAは会社がリスクアセスメントを実施して対処することを期待していると思われる。
EMAはどうかというと、昨年のNon-Cpmpliance reportを見る限り、FDAと同様の結果だった。こちらの方も「リスク」という言葉が頻繁に出てくるが、リスクマネジメントを導入していないとか、リスクアセスメントを実施していない、または実施が不十分という指摘はない。しかし、当局の追加コメントとして、リスクアセスメントをすることという記載は見られる。
“National competent authorities should request marketing authorisation holders conduct a risk assessment to evaluate the risk to product quality and patients from the issues identified. The risk assessments should contain sufficient information to inform decisions by competent authorities regarding product recall or the acceptability of continued testing for critical products."
すなわち、欧米では「リスクマネジメント」の導入不備や「リスクアセスメント」の不十分な実践で直接指摘はされないものの、間接的にリスクアセスメントを（強く？）求めていることが分かる。
